THE HEALTHY HEART SCREEN

PLEASE NOTE: This questionnaire will much more thoroughly evaluate your risk for cardiovascular disease beyond what is typically done in a conventional medical office.  You will notice that standard LDL cholesterol is not included. This is because research shows that approx 50% of people hospitalized with cardiovascular disease (plaque in arteries) have LDL cholesterol of less than 100 mg/dL, making it highly unreliable as a predictive marker for this condition and thus risk for heart attacks and strokes.  If you do not currently have access to some or all of the tests markers below, CONTACT US, to help you find the best option to get them done.

Please answer 'Yes' or 'No' for each question.

  1. I have a family history of diagnosed cardiovascular disease, heart attack, or stroke occurring before age 60.

  2. My fasting triglyceride level is higher than 150 ml/dl.

  3. My ApoB to ApoA-1 ratio is > 0.6.

  4. My homocysteine is higher than 10 micromol/L.

  5. My Lp(a) is higher than 30 mg/dL.

  6. My hs-CRP is higher than 1.0 mg/L.

  7. My fasting insulin is higher than 12 microunits/L.

  8. My HgA1c is > 5.6%

  9. I have an ApoE 3/4 or 4/4 genotype.

  10. My resting blood pressure is above 135/85

**Please note- It is our strong recommendation that anyone over the age of 40, scoring above the minimum risk level should obtain a carotid artery/CIMT scan as soon possible. If that test reveals arterial health status of greater than your current age, and/or the presence of plaque, please schedule a free consultation right away to help determine your best course of action.
